ACCESS BRIEF SCREENINGS

Brief screens in Access are done for individuals requesting treatment. These screenings are the first step to beginning services at LifeWays. In FY 2025, 3,757 brief screens were completed.

ANONYMOUS ONLINE SCREENINGS

LifeWays offers an anonymous online mental health screening on their website. This screening directs individuals to services once completed. Since beginning a new screening in May 2025, 284 online screenings were completed. The following are the top 4 screenings completed.

Integrated Health Improves Lives

ABOUT INTEGRATED HEALTH

LifeWays’ Integrated Health Clinic provides personalized care services that include:

  • A supportive care team working with each other on behalf of the consumer

  • Special attention for each consumer’s specific needs

  • Education for consumer’s on how to manage their health conditions

  • Care coordination

  • Behavioral health care

  • Primary Medical Care Services (physicals, sick visits, chronic care, etc.)

  • Resource navigation

Program availability: Available to anyone receiving services with LifeWays or its service providers.

PRIMARY CARE PHYSICIAN CLINIC VISITS

SCREENINGS COMPLETED

The Integrated Health Clinic uses Social Determinants of Health (SDoH) screenings. This screening looks at the nonmedical factors that influence health outcomes.

LifeWays periodically surveys its consumers to determine their level of satisfaction with services. LifeWays also receives survey data from a standardized survey that is distributed by the provider network to consumers they are serving.
