HABILITATION SUPPORTS WAIVER
(HSW)

What is HSW?

The Habilitation Supports Waiver Program (HSW) is an intensive home and community based support program designed to assist individuals with severe developmental disabilities to live independently, with supports, in their community of choice.

  • Out of Home Nonvocational Habilitation

The Process

  • To start the process, contact LifeWays or your Case Manager/Supports Coordinator and request an assessment for the Habilitation Supports Waiver Program.

  • Your Case Manager or Supports Coordinator will work with you and your supports to identify needs and develop a treatment plan. They are responsible for completing the necessary forms for submission.

    An HSW Coordinator will submit your potential enrollment packet to the Prepaid Inpatient Health Plan (PIHP) for consideration. The Michigan Department of Health and Human Services (MDHHS) approves or denies enrollment.

  • If approved for the HSW, an Individual Plan of Service is developed through the Person-Centered Planning Process, that reflects the individual’s needs.

REMINDERS

  • The individual must meet all of the eligibility requirements.

  • They must have documented habilitative needs in treatment plan.

  • They also must have medical necessity for at least one of the services available.

ELIGIBILITY

The individual seeking services must meet all the following requirements:

  • Has a developmental disability (as deemed by Michigan law) verified through testing or medical documentation.

  • Has current Medicaid eligibility. If there is a Medicaid deductible, this person is not eligible until the deductible is met each month.

  • Lives in a community-based setting (licensed setting, family, home, own home or apartment).

  • If not for the availability and provision for the HSW services, would otherwise require level of care services provided in an Intermediate Care Facility for Individuals with Intellectual and Developmental Disabilities (ICF/IDD.)

  • Chooses to participate in the HSW service array in lieu of ICF/IDD services.

  • Habilitation services under HSW are not otherwise available through a local educational agency under Individuals with Disabilities Education Act or the Rehabilitation Act of 1973.

  • Must not be simultaneously in any other 1915(c) waiver.

NOTE:

HSW beneficiaries must be enrolled through the Michigan Department of Health and Human Services (MDHHS) enrollment process complete by the Prepaid Inpatient Health Plan (PIHP). This enrollment process includes initial and annual verification of the above eligibility criteria.
